Once your doctor agrees to your recommendation, you will need to have them complete the “Physicians Certification” form stating which conditions qualify you for the use of medical marijuana. This information can be found in SECTION D – CERTIFYING PHYSICIAN INFORMATION of the Michigan Medical Marijuana Packet.
Second, you will need to complete the “Registry Application” form with all of your current accurate information. Be sure to completely fill out the application with truthful info. This information needs to be entered into SECTION A – APPLICANT/PATIENT information of the Michigan Medical Marijuana Packet
Third, you will need to write a check in the amount of $100 payable to “State of Michigan – MMMP”. If you are enrolled in Medicare, this amount is reduced to $25.
This step is only required if you will be designating a primary caregiver at the time of application. You will need to consider whether you want to designate an individual as your primary caregiver making them solely responsible for supplying you with marijuana or if you will be growing your own marijuana. If designating a primary caregiver see SECTION B – PRIMARY CAREGIVER. If growing your own medical marijuana see SECTION C – PERSON ALLOWED TO POSSESS PATIENT’S MARIJUANA PLANTS.
The next step is signing and dating the application at the bottom which basically authorizes the Michigan Department of Community Health to release your name and birth date to law enforcement only if the officer has provided the MDCH with your unique valid registration number.
